Evan Smoak a été enrôlé dès son plus jeune âge dans un programme secret consistant à former des orphelins aux techniques les plus efficaces d'élimination physique des ennemis de l'État lorsque le recours à la force n'est pas officiellement possible. Il a perdu son identité pour devenir « Orphelin X », mais il n'a pas perdu toute son humanité. Pendant des années, il a obéi aux ordres de son instructeur, il a agi en solitaire à la « neutralisation » de crapules et de terroristes. Pourtant, lorsqu'il accepte de s'occuper d'une jeune femme ayant contracté une importante dette de jeu et dont le père a été kidnappé, il est loin d'imaginer dans quel engrenage mortel il vient à son tour de mettre le doigt.
